Daycare Enrollment Application and Policies Agreement Downloads Below

• Daycare and group play require owner’s consent. 
• Daycare Enrollment Application and Policies Agreement must be completed, signed and on file PRIOR to evaluation. We require a recent copy of your pet's vaccine records to be on file.
• We required an evaluation of your dog’s temperament that includes a free ½ day of daycare.
• Evaluations are by appointment only. We require a completed Daycare Enrollment Application and Policies Agreement to be on file along with a recent copy of your pet's vaccine records, then we will reach out to you to schedule a date for your pet to come in for their evaluation.
• Tours are by appointment only. Email us to schedule your tour! 

• Due to an extremely high demand for daycare services we require a reservation be made in advance around the dates listed for daycare closure for your pet's day of play.
• We cannot guarantee availability for daycare without a reservation. 
• We recommend calling to reserve daycare dates as soon as possible to ensure that we are able to accommodate your needs. 

 All of our canine guests are required to have the following vaccinations:

• Rabies
• D/H/L/P
• Bordetella
• H3N2 *Not required, however it is recommended during high volume boarding periods 

 All pets MUST be healthy for the last 14 days prior to daycare, boarding or grooming. Any pet that is actively vomiting, having diarrhea, has recently started a new medication for seizures or is currently on medication for vomiting or diarrhea symptoms is NOT eligible to come for daycare, boarding or grooming at our facility. WE DO NOT ACCEPT ANY PET(s) with taped ears, ears or tails healing from a recent crop, OR with stitches anywhere on the body. Any pet(s) older than 10 years of age should receive a wellness check at their primary veterinary clinic the week of daycare, boarding or grooming to make sure the pet is healthy enough for their appointment or reservation. Pets that are unwell upon check-in will not be permitted to stay for boarding, daycare or grooming.  

If a pet is found to have a flea or tick infestation (4 or more parasites seen) we will REQUIRE that the pet receive a flea and tick shampoo treatment. If a client should refuse to pay for the flea and tick shampoo treatment they will incur an additional fee and the pet owner will be required to pick up the pet ASAP.   

• Full day daycare packages are not eligible for half day use
• Packages are non-refundable, non-transferable and expire after 1 year from the purchase date 

Daycare will be closed around Presidents Day, Spring Break, Fourth of July Thanksgiving, Christmas, and New Years Eve/Day. Please call for availability during those peak times We will give advanced notice prior to closing daycare. 

Lake Forest Kennel Club reserves the right to refuse any services for pets that do not meet the above requirements.
